


Before Your Medicare Plan Review
To help us make the most of your appointment, please have the following information available:
Your current plan information — Have your current Medicare Advantage, Medicare Supplement, and/or Prescription Drug Plan information available. Your Summary of Benefits or Annual Notice of Change (ANOC) is especially helpful.
Your medications — Make a list of your current prescription medications, including the name, dosage, and how often you take each medication.
Your preferred pharmacy — Include the pharmacy you normally use and any mail-order pharmacy you prefer.
Your doctors and specialists — Make a list of the physicians, specialists, hospitals, and other healthcare providers that are important for you to continue seeing.
Your coverage priorities — Think about what matters most to you, such as prescription coverage, keeping your doctors, hospital access, premiums, copays, maximum out-of-pocket costs, dental, vision, hearing, or other benefits.
Other people involved in your decision — If a spouse, family member, caregiver, Power of Attorney, or other trusted person helps you make healthcare decisions, you may want them available for the appointment.
Questions or concerns — Write down anything you dislike about your current coverage or anything you want to understand better.
Please do not send your Medicare number, Social Security number, banking information, or other sensitive personal information through this website form. If information is needed later to complete an enrollment, your licensed insurance agent will explain the appropriate process.
